LMS: z = ((measurement / M)^L − 1) / (L × S); for L = 0, z = ln(measurement / M) / S. Percentile = 100 × Φ(z).
WHO weight tails beyond ±3 use the WHO restricted LMS extension. CDC BMI above P95 uses percentile = 90 + 10 × Φ((BMI − P95) / sigma), then z = Φ⁻¹(percentile / 100).
CDC measurement review uses modified z = (measurement − M) / SD distance. SD distance is half the distance from M to the LMS value at +2 or −2, on the measurement’s side of M. Review flags: weight below −5 or above 8; stature below −5 or above 4; BMI below −4 or above 8. These are measurement-review limits, not diagnoses.
Age is chronological. WHO uses entered exact days; CDC uses the published half-month row for each month, with its exact 24-month boundary row. Measurements are never adjusted between standing and recumbent positions automatically. No corrected age, preterm curves, infant BMI classification, or diagnosis is supplied.
